To our patients
We are excited to announce that we are moving to a new location on February 05, 2024. Our new location address is: 12210 Plum Orchard Drive Suite 212, Sillver Spring, MD 20904.
Information for our patients
Anyone entering the office and is not feeling well or has a cough, sore throat, sinus congestion or fever must have a face mask covering their nose and mouth. If you do not have a proper face mask, you will not be allowed entry into the office.
New patients need to arrive at least 30 minutes prior to their appointment time in order to complete the registration process and to be seen by the doctor on time. Please bring your insurance card(s) and a photo ID with you. If patient would like to complete the new patient paperwork in advance and bring it with them to their appointment will be great.
Schedule a yearly visit. Patients who are not seen within a three year period will be considered inactive.
