We are happy to see self-pay patients , payment is expected at the time of service and it will be collected by the check -in desk. Any labs orders by the provider will be paid just before processing the order. For our patients that have medical insurance we will process your claims with our participating plans. We ask that you present your insurance card, photo identification, and any applicable copayment to the check-in desk when you arrive for your visit.

If you have an HMO, or POS insurance plan that requires a PCP, your insurance card must have our practice name, or one of our physicians, imprinted on the front of your card. If our name is not on your insurance card, unfortunately we will not be able to see you until it is rectified.

New Patients

If you are a new patient, you need to arrive 30 minutes before your appointment to complete your registration forms. If you are scheduling a complete physical exam (CPE), please read the CPE Waiver located in “Office Forms” to clarify our CPE policy.

Cancellation of Appointment

If there is a need to change or cancel an appointment, please notify us 24 hours prior to your scheduled time. There is a $50 charge for missed appointments.

Referral Requests

Referrals are generated during business hours and require a 48-hour advance notice. Please request a referral at the time of your visit. If you need a referral by phone, contact us with the appropriate information. We will prepare the referral for you to pick up, or we will mail it to you.
